Support Practices
Support will be delivered through an agreed individual support plan and delivery is based on Apex’s quality practice and the General Social Care Council code of practice.
Support plans will be person centred and focused on service users wishes and aspirations. Service users will be fully involved in the process to plan and participate in all areas of their lives. If they choose to do so, they can involve participation from family, friends and other relevant agencies in planning.
Support plans will be formally reviewed as a minimum on a 6 monthly basis however to ensure service user satisfaction and to meet any necessary change to support needs quarterly meetings with keyworkers will take place.
People shall be enabled to look after themselves and take responsibility for their own lives in as much as they are able to do so. The approach taken by support staff to achieve this shall include:
- Enabling and empowering people to do as much as possible for themselves by assisting people to obtain essential life skills.
- To promote independence through the provision of housing related support.
- Providing people with information and advice to enable them to make informed decisions.
- Using forms of communication appropriate to individuals, in terms of ability and culture, to ensure maximum understanding and involvement in how their support shall be provided.
- Ensuring that people have control over their personal money and possessions.
