An Overview of our Delivery of Care

In order to be able to deliver care Apex Care has developed an organisational structure with clearly defined roles and responsibilities and a range of Policies and Procedures that set out its aims and objectives and method of operation. These policies cover the delivery of care from the initial referral through to the review of the care package provided to the service user that encompasses the following.

The person responsible for the process from initial referral through to re-assessment of need is the Deputy Manager in conjunction with the Care Manager.

Assessment

It is the Deputy Manager who will obtain the basic details about the service that is being requested. This will include;

The days, times and number of visits required

The area in which the Service User lives

Detail of their needs and tasks required

Clearance from the Care Manager will be sought to ensure that capacity exists prior to accepting the referral.

Prior to the care package commencing, a visit to see the service user and/or family member will be arranged by the Deputy Manager with the purpose of obtaining and giving further information, relative to the services requested. This will include;

Provision of information about Apex Care in the form of a service user's Guide.

A service user care needs assessment will ascertain the wishes of the service user and will detail next of kin, days times and length of visits, other supporting services, background and any relevant medical details of the service user, cultural or religious needs, abilities of the service user and details of the tasks to be carried out.

A Basic Risk Assessment and if necessary a Moving and Handling Risk Assessment will be conducted by the Deputy Manager.

Copies of the assessment/care plan will be placed in the service user's Guide and left in the service user's house for reference by the Care Worker along with a medication chart if required.

An Apex Care service user Log Sheet will also be left with the service user in the service user's Guide folder. This will be used by the Care Worker to record information about their visit and a summary of tasks carried out and provides information for other Care Workers, relatives, Doctors and Health Visitors.

Once the service has commenced the service user, relative and/or Social Worker can contact the Deputy Manager in order to adjust any of the above requirements so long as it is within the ability of Apex Care to do so.

Assignment of Care Workers

Following finalisation of Contract/Agreement details, that the service user and Apex Care have agreed, the care service will commence, and suitable Care Workers will be assigned. The process of choosing the Care Worker will involve a sensitive review of the service user's overall care needs and wants.

In selecting a Care Worker, and wherever possible, the needs of the individual will be taken into account in allocating a preferred Care Worker.

Wherever possible, more than one preferred Care Worker will be matched with the service user to provide consistency of care and as a contingency measure in cases of staff absences.

The service user retains the right to request a change in the Care Worker at any time. Where such changes are requested, the service user will be advised to contact the Deputy Manager, and attempts will be made to accommodate such requests.

Review of the Care Plan

A review of the Care Plan including a Risk Assessment will be conducted annually by the responsible Deputy Manager, checked by the Care Manager and the Care Plan amended accordingly. The Social Worker will be informed as necessary.
